Waiver Form - PLAYLAND LEDUC
Release of Liability and Assumption of Risk
I, the undersigned, understand and acknowledge that participation in activities at 2357716 Alberta Ltd, carrying on business as PLAYLAND LEDUC (hereinafter “PLAYLAND LEDUC”) indoor playground may involve physical activity that carries a risk of injury, including but not limited to bumps, bruises, sprains, falls, and more serious injuries.
I acknowledge that:
- The indoor playground is designed for children’s play but no play environment is entirely risk-free.
- It is my responsibility to supervise my children at all times or ensure appropriate supervision while on the premises.
- I am voluntarily allowing my children to use the facilities and engage in activities at PLAYLAND LEDUC.
Waiver and Release of Liability
In consideration of being allowed to use the play facility and participate in related activities, I hereby release, waive, discharge, and covenant not to sue PLAYLAND LEDUC, its owners, agents, employees, volunteers, and representatives from any and all claims, liabilities, demands, actions or causes of action resulting from any injury, property damage, or loss that may occur while participating in activities at the facility, whether caused by negligence or otherwise.
Indemnification
I agree to indemnify and hold harmless PLAYLAND LEDUC and its affiliates from any claims, demands, losses, causes of action, damage, lawsuits, and judgments, including attorney’s fees and costs, arising out of my or my children’s participation in activities at the facility.
Medical Treatment
I understand that in the event of an injury, staff may provide basic first aid and/or contact emergency services as needed. I authorize emergency medical treatment for my children if necessary.
I consent do not consent to PLAYLAND LEDUC taking photographs or videos of my children during play for promotional or marketing purposes.
Acknowledgment
I have read and understand this Liability Waiver & Release Form. I understand that by signing this form, I am giving up certain legal rights on behalf of myself and my children. I certify that I am the parent or legal guardian of the children named above and that I have the authority to sign this agreement.
